Concerns.

If you are unhappy with any aspect of the service, please contact me and I will be happy to discuss your concerns and offer suggestions to resolve any issues.

Discharge and Termination of Contract

This usually occurs after an agreed period of intervention, when Therapy Plan Goals have been achieved or as/when parents and Therapist mutually agree. If a parent/client wishes to terminate the contract at any time, at least 1 months' notice by email is required.

Cancellation of appointments.

At least 24 hours notice of cancellation is required or the session fee will be charged, except in exceptional circumstances. If the therapist is unable to attend an appointment, the maximum possible notice will be given and the appointment will be rescheduled.

Other relevant professional involvement.

Please inform me if your child is currently receiving, or on a waiting list for, NHS or private SLT therapy, occupational therapy, educational psychology etc. as I will require your agreement for multi-disciplinary liaison, to ensure a holistic approach to caring for your child, and this includes sharing of reports and plans for intervention between involved professionals, including teaching staff.

Disclosure and Barring Service (DBS).

I hold an Enhanced Certificate from the DBS, maintain up to date 'Safeguarding' training and follow Government guidelines relating to child protection issues (www.gov.uk)
